When you respond online here, by phone, or by mail, be sure to count everyone who was living in your home on April 1st. You can respond online or by phone in English or 12 other languages. Households that have not responded online or by phone should have received a paper questionnaire.

A current utility bill, tax document, bank statement, credit card statement or car registration?
A driver’s license or state ID with a Jenkintown address?
Your child’s birth certificate or passport?
Up-to-date proof of immunization for your child?
